An informational packet -- including maps, health insurance instructions will be mailed to patients prior to their first appointment. When you schedule your appointment, you will be asked to register the patient with Patient Registration Services. You may, if you wish, Register On-line if your appointment is more than 48 hours away. Patients are advised to contact their health insurance provider prior to making an appointment in order to determine whether or not a formal referral from their primary-care physician is required.
All medical records, test results and X-rays pertinent to your condition should be brought to this first appointment, which may require up to one hour of time. Referring physicians offices may fax information about prior history and reasons for referral before the appointment.
